Respiratory Virus Checklist
Before you come in for your appointment, we ask that you read through the checklist below. We will also go through this checklist when you are scheduled for an appointment and at the reminder call.

Seattle Clinical Research Center Respiratory Virus Infection Prevention Policy
All patients will also be asked to wear a surgical mask when in the office. If you do not have one, we have masks that we can provide at the time of your appointment check in. All staff are masked, and we ask that you also wear a mask.

Pre-Visit Questionnaire
- Fever or chills
- Cough
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache
- Recent loss of taste or smell
- Sore throat
- Congestion
- Nausea or vomiting
- Diarrhea
2. Within the past 10 days, have you had contact with anyone that you know had cold or flu-like symptoms?
Contact is being 6 feet (2 meters) or closer for more than 15 minutes with a person or having direct contact with fluids from a person who is ill from a respiratory virus (for example, being coughed or sneezed on).
3. Have you had a positive test for a respiratory virus, such as influenza, RSV or COVID-19, in the past 10 days?
4. Within the past 7 days, has a public health or medical professional told you to self-monitor, self-isolate, or self-quarantine because of concerns about a respiratory virus infection?
